TERMS

If you {name} wish to cancel this contract, you may cancel by mailing or hand-delivering a written notice by certified or registered mail to the school. The notice must say that you do not wish to be bound by this contract and must be delivered or mailed before midnight of the third business day after you sign this contract. The notice must be delivered or mailed to 3 Wolcott Avenue, Lawrence, Ma 01843

All cancelation notices must be provided 30 days prior to the next billing cycle, if 30 days are not provided Diaz Isshinryu Karate and Kobudo reserve the right to withdraw payment. You may cancel this contract if you relocate your residence further than 25 miles from the school operated by Diaz Isshinryu Karate and Kobudo. This contract may also be canceled in case of death, or if the school ceases operation at the location where you entered into this contract. If you become disabled, you shall have the option of (1) being relieved of liability for payment on that portion of the contract term for which you are disabled by paying a cancellation fee for an amount equal to 50% of the remaining contract, or (2) extending the duration of the original contract at no cost to you for a period equal to the duration of the disability. You must prove such disability by a doctor’s certificate, which shall be enclosed with the written notice of disability sent to the school. *Any payments not received within two weeks (14 days) of the above collection date will result in a $25 Late Fee, which may be drawn automatically if applicable. An extended delay of tuition payment may result in further collection procedures and/or legal action. *If a check is returned by the bank there will be a $30 Non-Sufficient Funds Fee added on. Late payments to memberships are subject to a $20 late fee after 5 days from the due date. Membership payments can only be put on hold for medical reasons only with a doctor's note, hold will not be granted for vacations or breaks.

* Diaz Isshinryu Karate and Kobudo reserve the right to alter the days and/or hours of operation, the curriculum, and the number of classes you're entitled to attend, if deemed necessary Diaz Isshinryu Karate and Kobudo does not guarantee that you will reach a certain level of competence within any specific time frame and that students' obtaining a certain defined level of competence may be either delayed or accelerated due to changes in the curriculum or as deemed necessary by Diaz Isshinryu Karate and Kobudo.* The student and co-signer agree unconditionally to pay the above tuition without regard to the attendance made or classes missed until paid in full.

Release and Waiver of Liability and Media Release

We, the student {name} & guarantor, if applicable, on behalf of ourselves, members of our family, our heirs, executors, administrators and assigns, hereby forever release, discharge, and hold harmless by Diaz Isshinryu Karate and Kobudo representatives and agents for any injury, loss, or damage to my person or property howsoever caused, arising out of or in connection with my taking part in martial arts classes and activities and notwithstanding that the same may have been contributed to or occasioned by the negligence of by Diaz Isshinryu Karate and Kobudo Representatives or agents. Please Note: Participants must purchase their own protective equipment through Diaz Isshinryu Karate and Kobudo due to insurance purposes.

Lastly, I {name} grant Diaz Isshinryu Karate & Kobudo consent to use photos and videos taken in classes and events on social media and websites for the dojo.
